Found this on another site, and thought it was worth sharing:
  1. Alcohol can and will kill me, because I know I can’t stop once I start.
  2. There's a higher power that wants me to live. (My loved ones want that, too.) 
  3.  Do I want to live or die? (If I want to die, start praying until that ceases.)
  4. Write about how I got to where I am now, but don’t bother if I am not going to be completely honest. 
  5.  Share with another person what I wrote about me, both my good and my bad, (and ask that God listens, too). 
  6.  Decide now that I want to change my harmful habits and to grow up, both emotionally and spiritually. (If not now, when?) 
  7.  Ask a power greater than me to help me change. (I must do the work, but He will guide me, when I ask.) 
  8.  Write down who I've hurt, who I have sinned against; whether physically and/or emotionally. (Be sure to put myself on that list.) 
  9.  Fix what I can without hurting anyone else. (Learn why I hurt each of them and how not to do it again.) 
  10.  Accept myself, that as a human being, I will sometimes screw up; so learn from and fix my mistakes immediately. 
  11.  Always ask my Higher Power to show me how to live right so I can be comfortable in my own skin. 
  12.  To become a spiritual being, keep repeating steps 1 through 11 throughout my life and pass it on about what I have learned.
